In an effort to encourage good dental health and alleviate rising dental costs, We have developed an "In-House" Dental Savings Plan for our patients without dental insurance.

Our dental savings plan provides a wide scope of benefits at an affordable price to help minimize the cost of your dental needs. It also offers portability as you are not tied into any one employer. This is your plan and it follows you regardless of who you or your spouse works for during the year.

This is NOT dental Insurance. Enrolling in our In-House Dental Savings Plan gives you the opportunity to obtain your preventative services and dental treatments at our practice for reduced fees. It cannot be combined with dental insurance or other reduced fee denial.

ADULT PROGRAM $425

Includes:

•	Adult Periodic Exams (2 per year)
•	Routine Cleanings (2 per year)
•	Check up X-rays (1per year)
•	Fluoride Treatment (1per year)
•	15% off all dental procedures
•	Emergency Exams(during office hours)

CHILDREN'S PROGRAM $295

AGE 3-14 (You save $128 on Cleanings)

Includes:

•	Periodic Exams (2 per year)
•	Routine Cleanings (2 per year)
•	Check up X-rays (1per year)
•	Fluoride Treatment (1per year)
•	Emergency Exams(during office hours)
•	15% off all dental procedures

BENIFITS AND PROVISIONS OF THE IN-HOUSE DENTAL SAVINGS PLAN

•	No Annual Deductibles
•	No Annual Maximums
•	No Preauthorizations Required
•	No waiting for Period on Eligibility date
•	No Missing tooth clause or restrictions
•	Annual Enrollment fee is due at time of enrollment
•	Enrollment date begins on date of purchase, and it expires after 365 days.
•	Enrollment fee is non-refundable
•	To maximize savings, all family members should enroll. Every family member is eligible. No limits.
•	Our program is not transferable to another party or family member.
•	Participation cannot be combined with any other offer or dental plan.
